We are looking for Servers in USA
Specs required are:
Dual Core/ Quad core
4GB RAM
100Mbps LAN
100-200GB HDD
Our budget is $12-15/month. Please recommend us companies/Suppliers.

With your budget I would go with VPS instead. The only dedicated server provider came in mind with that budget is WholesaleInternet.

Agreed that your best bet is to get a decent VPS. At $12-15 monthly you cannot expect much at all out of a physical dedicated server. That could maybe cover the electricity and bandwidth, assuming it's a very small allocation.

I guess Vultr is your best bet then, here's some benchmark test result I ran on their bandwidth of 1Gbps earlier today
100mb.test 100%[=====================>] 100.00M 127MB/s in 0.8s
2016-10-17 15:42:30 (127 MB/s) - ‘100mb.test’ saved [104857600/104857600]
